ABSTRACT

ObjectiveTo understand the situation of nosocomial infection in cancer hospitals and its changing trend, so as to provide a basis for adjusting the focus of nosocomial infection prevention and control in cancer hospitals. MethodsData of nosocomial infection quality control indices of Sun Yat-sen University Cancer Center from 2019 to 2021 were obtained through the nosocomial infection monitoring system, and the changes of these indices across the three years were analyzed by Chi-square test and Cochran-Armitage trend test. ResultsFrom 2019 to 2021, the incidence rates of nosocomial infection in this hospital were 0.80%, 0.78% and 0.57%, which decreased significantly year by year (P<0.001). Among them, surgical site and respiratory system infection were more common, accounting for 35.75% and 31.08%, respectively. Gram-negative bacteria and fungi were the main pathogens. The incidence rate of multidrug-resistant bacteria in hospital increased year by year, from 0.08‰ to 0.14‰ (P<0.001), among which methicillin-resistant staphylococcus aureus, carbapenem-resistant Enterobacter and bacteria producing ultra-broad spectrum β-lactamase (ESBLs) bacteria increased significantly. The incidence rates of three-tube associated infections were no different across 3 years (P>0.05), which were still at high levels. ConclusionFrom 2019 to 2021, the prevention and control of nonsocomial infection in the cancer hospital has been improved overall. Meanwhile, the infections of respiratory system and surgical sites, ESBLs related multidrug-resistant bacteria and three-tube are weak links in cancer specialized hospitals, which need to be emphasized and improved.

ABSTRACT

Objective:To explore the consistency of MRI-based ovarian-adnexal report and data system (O-RADS) score and its diagnostic value for ovarian adnexal masses. Methods:The MRI data of 309 patients with ovarian adnexal masses confirmed by pathology were retrospectively collected from January 2017 to August 2021 in the Second Affiliated Hospital of Soochow University, including 327 lesions consisted of 250 benign lesions, 21 borderline lesions, and 56 malignant lesions confirmed by pathology. Borderline and malignant lesions were classified into the malignant group ( n=77) and benign lesions were classified as benign group ( n=250). Two radiologists scored all lesions according to the MRI-based O-RADS, and scored again after 6 months. The proportion of borderline/malignant lesions in each MRI-based O-RADS score was calculated. The weighted Kappa test was used to assess the intra-reader and inter-reader consistency of the image interpretation results. The receiver operating characteristic (ROC) curve analysis was used to evaluate the diagnostic efficacy of MRI-based O-RADS classification for distinguishing benign and malignant ovarian adnexal masses. Results:The weighted Kappa value of the MRI-based O-RADS score between the two radiologists was 0.810 (95%CI 0.764-0.855), and the weighted Kappa values of the two radiologists′ scores at different times were 0.848 (95%CI 0.806-0.889) and 0.875 (95%CI 0.835-0.914), respectively. The borderline/malignant lesions accounted for 0/16, 0.8% (1/127), 10.1% (10/99), 76.0% (57/75), 9/10 and 0/17, 0 (0/122), 8.0% (8/100), 76.2% (48/63), and 84.0% (21/25) of the lesions in the two radiologists based on the MRI O-RADS score of 1, 2, 3, 4, and 5, respectively. When adopting O-RADS score>3 as a cut-off value, the area under the ROC curve of the two radiologists for distinguishing benign and malignant ovarian adnexal masses was 0.928 (95%CI 0.895-0.954) and 0.942 (95%CI 0.911-0.965), respectively. The sensitivity was 0.857 and 0.896, the specificity was 0.924 and 0.924, and the accuracy was 0.908 and 0.917 respectively. Conclusion:The MRI-based O-RADS yields high diagnostic efficiency in the evaluation of benign and malignant ovarian adnexal masses, and the intra-reader and inter-reader consistency of the image interpretation is strong.

ABSTRACT

Various academic discussions have been officially proposed in the " theory of circular motion" in Circular Motion of Ancient Chinese Medicine. The theory of circular motion reflects the inheritance and development of the theory of Yin-Yang and five elements in ancient Chinese medicine, which has a great significance in the modern basic theory research and clinical practice of traditional Chinese medicine. On the basis of ancient literatures, this article aims to discuss the natural law of circular motion from the perspectives of the path of sun and 24-solar-terms of seasons via five-elements and Wufang circular motion theory according to circular motion. The circular motion has an effect on human body and natural environment. The invasion of pathogen is caused by deficiency of essential Qi. The combination of the healthy-Qi deficiency with the deficiency-type pathogen leads to the occurrence of external contraction. Therefore, the abnormality of Yang-Qi including nature and human is the main cause of the disease's development. Because the circular motion can expound the change rule and mutual relations of nature and human, we can predict the development of disease by it. When the circulation motion is disordered, clinical symptoms appear with inner pathogenesis. The understanding of the change of the circulation motion is helpful for us to have a deeper realization of the disease, grasp the pathogenesis and provide treatment based on syndrome differentiation. The authors explain nature environment and human physiology, and elaborate the relations between natural rule and Wufang's effect on circulation motion, in order to provide certain suggestions to guide clinical treatment based on pathogenesis.

Objective To perform an exploratory investigation on confocal laser endomicroscopy (CLE) in the diagnosis of malignant bladder tumour. Methods From June 10 to July 11,2017,6 male bladder cancer patients underwent white light cystoscopy (WLC) + CLE examination, aging 64-86 years (median 72 years). All patients received TURBT on suspected lesions.WLC and CLE imaging results were recorded and validated by pathologic specimens. Results Lesions confirmed by histopathology were 3 low grade non-invasive papillary urothelial carcinomas,1 high grade non-invasive papillary urothelial carcinoma,1 low grade invasive urothelial carcinoma,1 high grade invasive urothelial carcinoma,1 carcinoma in situ (CIS),1 high grade dysplasia,1 cystitis glandularis,1 chronic inflammation, and 1 scar tissue. For CLE images in the normal urothelium, three layers of cells with different presentation were observed, namely, the superficial umbrella cells, the intermediate cells smaller in size and uniformly shaped, and the capillary network in the lamina propria. For non-invasive urothelial carcinoma, tumour cells appeared as papillary lesions growing from fibrovascular cores, with low grade cells appearing monomorphic and more cohesively arranged, and high grade cells relatively pleomorphic, more disorganised and with tortuous blood vessels in the fibrovascular core. For invasive urothelial carcinoma, tumour cells invaded the lamina propria, with uniform appearances, poor cohesion and indistinct cellular borders, and high grade ones were more pleomorphic.CIS and inflammation both appeared as erythematous patch-like flat lesions under WLC and sometimes difficult to differentiate.Under CLE,the former appeared as dysplastic and disorganised cells with indistinct cellular borders, with intact lamina propria, and inflammatory cells were discovered as infiltrative clusters in the lamina propria that were uniformly shaped and loosely connected.Dysplasia appeared somewhat similar compared with CIS under WLC,but with lower cellular irregularity as confirmed with pathology.Cellular appearance and structure in scar tissue was similar to that in the normal urothelium, but superficial umbrella cells were more likely absent, with thinner cell layers, and inflammatory infiltration was sometimes discovered in the lamina propria. Conclusions CLE provides real-time cellular imaging of the urothelium, and shows promising potential for clinical diagnosis, especially in differentiating fiat urothelial lesions.Large prospective studies are required for further validation.

ABSTRACT

To measure particle size distribution of phenols in mainstream cigarette smoke aerosol, the particles of cigarette smoke aerosol were divided into 12 stages using single channel smoking machine coupled electrical low-pressure impactor (ELPI) and collected by 12 polyester films. The collected particles were weighted and then analyzed by ultra-high performance liquid chromatography-fluorescence detection (UPLC-FLD) to determine the 14 phenols in the different size particles. The results showed that the aerosols collecting method had good stability with relative standard deviation (RSD) of collected particles mass less than 10%.The analyzing results of 14 phenols by UPLC-FLD showed that the linear correlation coefficients(R2) were greater than 0.9959,with detection limits were less than 1.2 ng/cig and recoveries were 80.1%-115.0%.The distributions of 14 phenols with respect to smoke aerosol particle size were investigated. The results indicated that except 4-ethyl-2-methoxy-phenol not detected, the other 13 phenols were detected in mainstream cigarette smoke aerosol. The content of 13 phenols appeared increasing at first and then decreasing with increase of the particle size which distributed in a pattern similar to that of particle mass. All of 13 phenols were present in higher amounts in the medium size particles (0.261-0.722 μm) with peak content in particles 0.431 μm.The distribution of concentrations (ratio of content to particle mass) of 13 phenols in different size particles was different. The concentrations of phenol and mono-substituted phenol appeared to first increase and then decrease with increasing smoke aerosol particle size and were higher in medium size particles (0.261-0.772 μm).The concentrations of benzenediol and mono-substituted benzenediol were uniformly distributed in medium size particles (0.144-1.166 μm),and the concentration of disubstituted phenol was uniform throughout the particles of varying sizes.

ABSTRACT

Ten isoprenylated flavonoids were isolated from 95% ethanol extraction of Artocarpus heterophyllus, with a combination of various chromatographic approaches, including ODS, MCI, CHP-20 P, Sephadex LH-20 and high performance liquid chromatography. On the basis of physic-chemical characters and spectroscopic data analysis, these compounds were identified as artoheteroid E (1), cycloheterophyllin (2), artelastoxanthone (3), artoindonesianin Q (4), cudraflavone C (5), 8-(γ,γ-dimethylallyl)-5,2',4'-trihydroxy-7-methoxyflavone (6), kuwanon T (7), 6-(3-methylbut-2-enyl) apigenin (8), 5,7,2',4'-tetrahydroxy-6-(3-methylbut-3-enyl) flavone (9), albanin A (10). Among them, compound 1 is a new one, while compounds 2-4 were isolated for the first time from the plant of Artocarpus heterophyllus. All isolated compounds were screened for their inhibitory abilities against cathepsin K. Of them, compounds 3-5, 7 and 10 showed inhibitory effects with the IC50 values of 0.9, 1.6, 4.5, 24.5 and 63.5 μmol·L-1, respectively.

ABSTRACT

OBJECTIVEMeta analysis was used to evaluate the efficacy and safety of Dynesys and posterior decompression and fusion internal fixation for lumbar degenerative diseases.

METHODSThe computer was used to retrieve the Cochrane library, Medline, Embase, CNKI, Wanfang database and Chinese biomedical literature database; and the references and main Chinese and English Department of orthopedics journals were manually searched. All the prospective or retrospective comparative studies on the clinical efficacy and safety of Dynesys and posterior decompression and fusion internal fixation were collected, so as to evaluate the methodological quality of the study and to extract the data. The RevMan 5.2 software provided by Cochrane collaboration was used for systematic evaluation.

RESULTSA total of 9 clinical studies were included, including 3 prospective randomized controlled trials(RCT) and 6 retrospective controlled observational studies, which included 692 patients, with 336 cases in Dynesys group, and 356 cases in posterior decompression and fusion internal fixation (PLIF) group. The results showed that compared with PLIF, Dynesys system significantly decreased operation time(<0.01), intraoperative blood loss (<0.01). Both Dynesys and PLIF groups experienced improved ODI and back/leg pain VAS scores at final follow-up, and no statistically significant difference was noted according to the two surgical procedures(>0.05). Dynesys could remain the range of motion (ROM) of surgical segments with less increased ROM of adjacent segments compared with that of PLIF group(<0.01). Regarding the disc height of surgical segments, no statistically significant difference was noted according to the two groups(>0.05), but postoperative complications incidence rate in PLIF group was higher than that in Dynesys group (<0.05).

CONCLUSIONSBoth Dynesys system and PLIF can improve clinical outcomes of lumbar degenerative diseases effectively. Compared with PLIF, Dynesys could remain the range of motion(ROM) of surgical segments with less increased ROM of adjacent segments and lower complication incidence rate. But the ability to prevent adjacent segments degeneration needs more RCTs with long-term follow-up to confirm.
